If you have a medical records request or need to release public health information, complete the appropriate form(s) by selecting the buttons. Feedback, Complaint & Appeal

At the Bert Nash Center, we strongly encourage all clients, their family members, or any community member to communicate any feedback, positive or negative, including a complaint/grievance, in person, in written format, via email, or by phone to the BNC Risk Manager.

The Feedback Form is a place where you can express concerns and/or complaints if you are not satisfied with the service you are receiving, if you feel your rights have been violated, or if you have talked with your mental health professional about your concerns and are dis-satisfied with their response.
